Katherine P. Cabral, Pharm.D., BCPS, CACP, is Assistant Professor of Pharmacy Practice at Albany College of Pharmacy and Health Sciences in Albany, New York. She also maintains a clinical practice with the cardiology services at Albany Medical Center.
Dr. Cabral earned her Doctor of Pharmacy degree from Northeastern University in Boston, Massachusetts. She completed a pharmacy practice residency, as well as a cardiology specialty residency, at Boston Medical Center. Dr. Cabral is a board-certified pharmacotherapy specialist and Certified Anticoagulation Care Provider.
Previously, Dr. Cabral practiced a clinical pharmacy specialist in anticoagulation at Maine Medical Center and Boston Medical Center. During her time in both positions, she was responsible for developing and implementing inpatient anticoagulation services. Dr. Cabral was also actively involved in quality improvement projects that focused on ensuring safe and effective use of antithrombotic agents. Dr. Cabral has taught at Massachusetts College of Pharmacy, Northeastern University, and the University of New England.
Dr. Cabral’s didactic responsibilities at the Albany College of Pharmacy include cardiovascular and antithrombotic topics. She has authored several articles on anticoagulant-related topics and is a reviewer for the American Journal of Health-System Pharmacy and the Journal of Thrombosis and Haemostasis. Dr. Cabral has served as an invited speaker nationally on optimizing anticoagulant therapy in acute coronary syndrome and in the prevention and treatment of venous thromboembolism.

William E. Dager, Pharm.D., FCSHP, FCCP, FCCM, is a board-certified pharmacotherapy specialist who currently holds three academic positions. He is Clinical Professor of Pharmacy at the University of California, San Francisco (UCSF) School of Pharmacy and Clinical Professor of Medicine at the University of California, Davis (UC Davis) School of Medicine. He also serves as Clinical Professor at the Touro School of Pharmacy in Vallejo, California. As a clinical specialist at UC Davis Medical Center, he is responsible for managing challenging cases in anticoagulation, pharmacokinetics, and critical care. Dr. Dager also is clinically active with the cardiology service and serves as the director of the postgraduate year two (PGY2) residency in cardiology at UC Davis.
Dr. Dager earned his Doctor of Pharmacy degree at UCSF and completed a residency at the UC Davis Medical Center in Sacramento. In addition, he completed the University of Pittsburgh Nephrology Pharmaceutical Care Preceptorship. He is a fellow of the California Society of Hospital Pharmacists, American College of Clinical Pharmacy (ACCP), and Society of Critical Care Medicine (SCCM).
Dr. Dager's research interests focus on anticoagulation, critical care medicine, cardiovascular disease, and pharmacokinetics and pharmacodynamics. He has authored numerous articles, book chapters, and scientific reviews, and he has made numerous presentations at national and international educational conferences. He serves as a reviewer and editorial board member for several medical journals, and he currently is chair of the Editorial Advisory Board Panel on Anticoagulation for The Annals of Pharmacotherapy. Dr. Dager also is a site coordinator for the American Society of Health-System Pharmacists (ASHP) Research and Education Foundation Antithrombotic Pharmacotherapy Traineeship.
Dr. Dager has received multiple teaching and professional awards, including the 2008 ACCP Best Practice Award. He is a member of numerous professional organizations, including ACCP, ASHP, Sacramento Valley Society of Health-System Pharmacists, Anticoagulation Forum, International Society of Thrombosis and Haemostasis, and SCCM. He also currently serves as an instructor and regional affiliate faculty member in advanced cardiac life support for the American Heart Association.

John Fanikos, M.B.A., R.Ph., is Director of Pharmacy Business at Brigham and Women's Hospital (BWH) in Boston, Massachusetts. Mr. Fanikos is also Assistant Professor of Clinical Pharmacy Practice at Northeastern University and at the Massachusetts College of Pharmacy (MCP), both of which are his alma maters. He earned his Bachelor of Science degree in pharmacy at MCP and his Master of Business Administration degree at Northeastern. He completed an American Society of Health-System Pharmacists - accredited residency at BWH.
Mr. Fanikos has served in a number of different roles at BWH, including investigational drug pharmacist, inpatient and outpatient pharmacy supervisor, clinical preceptor, and pharmacy administrator. While fulfilling these roles, he has worked with patients on orthopedic, oncology, cardiology, and general medicine units. For the past ten years, he has worked closely with the BWH Venous Thromboembolism Service, and he has authored and co-authored many articles on the complications of anticoagulation therapy. He serves on the Board of Directors for the North American Thrombosis Forum.
Mr. Fanikos currently serves as a pharmacist member of the National Comprehensive Cancer Network Venous Thromboembolic Disease Panel and on its Writing Committee. He is also President of the Massachusetts Society of Health-System Pharmacists, and a member of the Voluntary Hospital Association Northeast States Clinical Pharmacy Council.

Elizabeth Farrington, Pharm.D., BCPS, FCCM, FCCP, FPPAG, is Pharmacist III in Pediatrics at the New Hanover Regional Medical Center in Wilmington, North Carolina and Clinical Assistant Professor for the University of North Carolina Eshelman School of Pharmacy, Chapel Hill.
Dr. Farrington earned her Bachelor of Science degree in pharmacy from the University of North Carolina and her Doctor of Pharmacy degree from the University of Kentucky. She completed a two-year clinical pharmacy residency, including one year in pediatrics, at the University of Kentucky. She also completed a pediatric research fellowship at the University of Iowa. Dr. Farrington is a fellow in the Society of Critical Care Medicine, the American College of Clinical Pharmacy (ACCP), and the Pediatric Pharmacy Advocacy Group (PPAG). She is also a board certified pharmacotherapy specialist.
For the past twenty years, Dr. Farrington’s primary area of practice has been the Pediatric Intensive Care Unit. She has worked closely with nursing to implement standard concentrations and smart pump technology in the pediatric hospital. Dr. Farrington also directed a Pediatric Pharmacotherapy Residency and trained over fourteen residents. She has published peer-reviewed journal articles and book chapters in the field of pediatric pharmacotherapy and is invited nationally to speak on pediatric pharmacotherapy and drug safety topics. Dr. Farrington previously served on the PPAG Board of Directors and is currently a member of the ACCP Board of Regents.

Michael P. Gulseth, Pharm.D., BCPS is Program Director for Anticoagulation Services at the Sanford University of South Dakota Medical Center, Department of Pharmaceutical Services in Sioux Falls, South Dakota. Dr. Gulseth received his Doctor of Pharmacy degree in 1999 from North Dakota State University in Fargo, North Dakota. He completed residency training at United Hospital in St. Paul, Minnesota. He is lead author and editor of Managing Anticoagulation Patients in the Hospital: the Inpatient Anticoagulation Service, published in 2007 by the American Society of Health-System Pharmacists.
Dr. Gulseth is a member of the Editorial Advisory Board of the American Journal of Health-System Pharmacy. He is an active member of several professional pharmacy organizations, including the American Society of Health-System Pharmacists, the Minnesota Society of Health-System Pharmacists (past president), the South Dakota Society of Health-System Pharmacists, and the American College of Clinical Pharmacy. His research interests include evaluation of antithrombotic therapies in the inpatient setting and evaluation of the benefits of pharmacist-managed inpatient anticoagulation services.

Stuart T. Haines, Pharm.D. is Professor in the Department of Pharmacy Practice and Science at the University of Maryland School of Pharmacy in Baltimore, Maryland and Clinical Pharmacy Specialist at the West Palm Beach VA Medical Center in West Palm Beach, Florida. He is a Past-President of the American College of Clinical Pharmacy (ACCP), having served on the ACCP Board of Regents from 2002-2008. He is currently a member of the American Society of Health-System Pharmacists (ASHP) Commission on Credentialing – the body that sets standards and accredits pharmacy residency and technician training programs in the United States. He has served on numerous editorial boards including Pharmacotherapy, The Annals of Pharmacotherapy, Current Medical Research and Opinion, the American Journal of Health-System Pharmacy (AJHP), the Journal of the American Pharmacists Association (JAPhA), as well as the Pharmacist’s Letter.
Dr. Haines earned his Bachelor of Science degree in pharmacy from the Massachusetts College of Pharmacy and Allied Health Sciences and his Doctor of Pharmacy degree from the University of Texas at Austin. He completed a pharmacy practice residency at the Brigham and Women's Hospital in Boston, as well as an ambulatory care residency at the University of Texas Health Science Center in San Antonio. He is a board-certified pharmacotherapy specialist (BCPS), and he is also board-certified in advanced diabetes management (BC-ADM). Dr. Haines is a fellow in the American College of Clinical Pharmacy (FCCP), the American Society of Health-System Pharmacists (FASHP), and the American Pharmacists Association (FAPhA). In 2006, Dr. Haines was elected a Distinguished Practitioner in the National Academies of Practice.
Dr. Haines' scholarly interests include medication use behavior, cardiovascular disease prevention, diabetes management, women's health, and the evaluation of instructional methods.
Gregory A. Maynard, M.D., M.S., SFHM

Gregory A. Maynard, M.D., M.S., is Clinical Professor of Medicine and Chief of the Division of Hospital Medicine at the University of California, San Diego (UCSD). He earned his Doctor of Medicine degree at the University of Illinois and Master of Science degree in biostatistics and clinical research design from the University of Michigan.
Dr. Maynard is active on both a local and national level in many efforts to improve the quality and safety of the care delivered to inpatients. His special interests include optimizing prevention and management of venous thromboembolism (VTE), improving glycemic control and reducing hypoglycemia in the hospital, and transitions of care. Dr. Maynard’s research interests parallel his quality improvement efforts, and he is principal investigator or otherwise involved in several funded research efforts. He acts as a leader and mentor in national collaboratives to improve VTE prevention coordinated by the Society of Hospital Medicine (SHM), the Agency for Healthcare Research and Quality, the American Society of Health-System Pharmacists, and the Institute for Healthcare Improvement. He is a mentor / investigator for Project BOOST, an SHM initiative to improve transitions in care and is also a mentor / investigator for the SHM Glycemic Control collaborative. Dr. Maynard acted as the leader of the SHM Glycemic Control Task Force, generating the SHM Glycemic Control Resource Room and the supplement in the Journal of Hospital Medicine reflecting the task force work, and he has published numerous peer reviewed papers on the subject.
Dr. Maynard was recently recognized as one of ACP Hospitalist's top hospitalists, and as a San Diego “Top Doc”, and has been recognized nationally for his work in Quality Improvement / Research by the Society of Hospital Medicine, the National Association of Public Hospitals, the Venous Disease Coalition, and the North American Thrombosis Forum. He was recently named the Director of the UCSD Center for Innovation and Improvement Science.

Jordan C. Messler, M.D., FHM, is Medical Director, Morton Plant Hospitalists, Morton Plant Hospital in Clearwater, Florida. He earned his Doctor of Medicine degree at the University of South Florida College of Medicine in Tampa, Florida, and completed a residency in internal medicine at Emory University in Atlanta, Georgia.
Upon completion of his residency training, Dr. Messler served as faculty at Emory University, working as a hospitalist in one of the first academic center hospitalist groups. In 2005, Dr. Messler relocated to serve as the director of an active community hospitalist group at Morton Plant Hospital in Clearwater, Florida. He is a member of the 2009 inaugural class of Fellows in Hospital Medicine.
Dr. Messler is active in several quality improvement projects, leading hospital-wide efforts to improve prevention and management of venous thromboembolism (VTE), inpatient management of diabetes, and transitions of care. He participates in the following national initiatives coordinated by the Society of Hospital Medicine (SHM); including VTE prevention, project BOOST, designed to improve transitions in patient care, and a national initiative to improve glycemic control in patients with diabetes. In April 2010, Dr. Messler joined SHM’s Hospital Quality and Patient Safety Committee.

Lynn B. Oertel, M.S. ANP-C, CACP, is a clinical nurse specialist with Anticoagulation Management Services at Massachusetts General Hospital in Boston, Massachusetts. Massachusetts General Hospital has one of the largest and most experienced warfarin management clinics in the United States.
Ms. Oertel received her Master of Science in Nursing degree from Boston University. She has additional certification from the American Nurse’s Credentialing Center as an adult nurse practitioner. Ms. Oertel is a certified anticoagulation care provider (CACP) and is nationally recognized as an expert nurse clinician in anticoagulation therapy management. She has nearly 25 years of clinical and research experience related to anticoagulant therapy management.
Ms. Oertel is a founding member of the Board of Directors of the National Certification Board for Anticoagulation Providers, the only multidisciplinary national certification for anticoagulation care providers. She is currently a member of the Board of Directors of the Anticoagulation Forum and co-editor and chapter author of the third edition textbook Managing Oral Anticoagulation Therapy: Clinical and Operational Guidelines.

Zachary A. Stacy, Pharm.D., BCPS, is Associate Professor of Pharmacy Practice at the St. Louis College of Pharmacy. He earned his Bachelor of Science degrees in chemistry and biology from Blackburn College and his Masters in Chemistry degree from Southern Illinois University in Edwardsville. Dr. Stacy completed his Doctor of Pharmacy degree at the St. Louis College of Pharmacy and a specialty residency in internal medicine at St. Luke’s Hospital in Chesterfield, Missouri. He is a board-certified pharmacotherapy specialist. He maintains a clinical practice on the internal medicine and cardiology services at St. Luke’s Hospital in Chesterfield.
Dr. Stacy’s didactic responsibilities at the St. Louis College of Pharmacy include ischemic heart disease, hyperlipidemia, heart failure, peripheral arterial disease, and venous thromboembolism. He is an adjunct clinical professor in the Physician Assistant program at Saint Louis University and serves as an ACLS instructor. Dr. Stacy has received several teaching awards including the 2005 Excellence in Clinical Precepting Award, the 2009 Joe E. Haberle Outstanding Educator of the Year Award, and the 2010 Missouri Pharmacy Association Faculty Member of the Year Award.
Dr. Stacy’s primary research interests include the prevention of venous thromboembolism and the management of peripheral arterial disease. He has authored numerous articles, reviews, and book chapters on these topics and frequently provides presentations at state and national conferences. He is an active member of many pharmacy organizations including the American College of Clinical Pharmacy and the American Society of Health-System Pharmacists.
